Thanks for your interest in Opinionated About Dining. We publish a guide to dining in the United States based on a survey of knowledgeable and experienced diners. As an incentive to get you to participate in our 2011 survey, we have entered into a special arrangement with ChefShop.com whereby they will provide you with credits that can be worth up to $150, and which you can use towards the purchase of gourmet ingredients and specialty foods, when you rate restaurants for our survey in accordance with the Terms and Conditions set out below. Here is how it works::

1. Click on the link at the bottom of this page to register for the survey. Enter the word “olive" (without the quotes) as the pre-approval code.

2. In the box labeled How Did You Hear About Opinionated About, please enter ChefShop.com. We are not in possession of the ChefShop.com email list, and the only way we will able to know that you qualify for this promotion is if you tell us you were referred by ChefShop.com

3. Once you have access to the survey, rate as many restaurants as you can. The restaurants can be located in the U.S., Canada or Europe. Try and click on any link that might look like it covers a region that you have visited.

4. In order to qualify for the credits, you need to rate the following number of restaurants:

-----a) Rate any 50 restaurants and get a Chefshop,com store credit for $50.00 of gourmet ingredients or specialty food items

-----b) For each additional 25 restaurants that you rate, your store credit will be increased by $25.00

-----c) The maximum credit that can be earned under this promotion is $150.00

5. You must be at least eighteen (18) years old to participate in this promotion.

6. No prior purchase from ChefShop.com is necessary

7. Only ratings entered on or after March 15, 2010 will count towards your total. If your name was not on the original list of people who received the email from ChefShop.com, you are not eligible for this promotion.

8. Your ratings must be entered no later than 11:59 PM PCT on March 29, 2010 in order to qualify for the merchandise credit.

9. On March 31st we will provide ChefShop.com with a list of the people who qualified for a merchandise credit, along with amounts earned, in accordance the terms of this promotion. ChefShop.com will then contact to provide you with the information you will need in order to apply the credits towards purchases made on their site.

10. In addition to entering your ratings, we would appreciate it if you can enter a short review or comment about each restaurant. But please note that commenting is optional and in no way does it impact the amount of the credit you can earn.
